While playing a game in Futurama season 12, episode 2, Fry excitedly says that he hopes his grandmother is watching him from hell, meaning that she passed away at some point. However, the young Fry believing that Mildred is in hell means that he undoubtedly had a bad relationship with his grandmother, although very few details regarding this are given.
Futurama hasn't spent a lot of time developing Fry's grandmother, as the show hasn't revealed how involved she was in Fry's life or when she died.
